Is there a risk of filler migration or displacement after scar filler treatment?

The risk of filler migration (displacement) after scar filler treatment is largely related to the technique used, the targeted anatomical plane, and the properties of the injected material.
 
In microinjection techniques performed with a needle—particularly in the treatment of atrophic scars—hyaluronic acid is placed at the superficial dermal level in very small volumes. The primary goal of this approach is to elevate the scar surface to match the level of the surrounding healthy skin and to provide localized structural support. At this superficial dermal plane, the injected filler is not expected to spread over wide areas or migrate. Therefore, when superficial scar filler treatment is performed with proper technique, there is no risk of filler migration.
 
In contrast, when injections are performed using a cannula in deeper planes—especially at the level of the subcutaneous fat—and with higher volumes, there is a potential for the material to move within the surrounding tissues. This risk is particularly relevant in fat injections and in applications involving high-viscosity fillers or larger volumes, where increased regional fullness may contribute to migration.
 
In conclusion, the risk of filler migration after scar treatment is primarily determined by the injection technique, the depth of application, and the characteristics of the material used. In superficial dermal, low-volume, microinjection-based approaches, the risk of migration is not present.
